A withdrawal report/unofficial transcript from prior School is helpful.

All students new to the Prague School District must enroll in person at the office.

You may download and complete the following forms or complete them at the office.

Elementary Enrollment

The following documents are required for initial enrollment:

  • Certified Birth Certificate

  • Immunization Record

  • SS card/number

  • Custody documents

Login and complete the annual demographic review and forms for current Prague Public School students.
